Police and EMS were called to the scene of a man in his mid-twenties with a past medical history of ADHD. At the scene, the patient was found to be extremely agitated with altered mental status. He was shot multiple times with tasers by the police in order to be subdued.

A female in her 20s presented to the ED for worsening right upper quadrant abdominal pain, nausea/vomiting, malaise, fever/chills, and jaundice. She has no significant past medical history, however, was recently seen for similar symptoms a few days prior. At that time, she had bloodwork done that was diagnostic for acute infectious mononucleosis, including a positive monospot test and IgM Epstein-Barr virus (EBV) antibodies (with negative IgG EBV antibodies), and mildly elevated AST, ALT, and bilirubin.

A boy aged 7 years was investigated for dizziness, diplopia and occasional visual hallucinations over a period of three months. Examination revealed mild nystagmus and left-sided cerebellar signs, including ataxia and dysdiadochokinesis. A mass arising from the roof of the fourth ventricle was demonstrated on MRI (Fig. 1). No other radiologic abnormalities were present within the neuraxis. Serum AFP was 1KU/L, and HCG was < 1 IU/L. The tumor was removed via a posterior fossa craniotomy. The child received craniospinal radiotherapy and cisplatin-based chemotherapy, and remains well 44 months post-surgery.

An 18 year-old woman presented to the emergency room with a five-day history of right upper abdominal pain accompanied by nausea with hematemesis, generalized malaise, cephalgia without provocation, fever, and diarrhea. She was a non-smoker, non-drinker and was not taking any medications (prescribed, over-the-counter, or supplements).

A 16-month-old boy presented to his pediatrician following a 3 week history of left head tilt and a 3 day history of cessation of crawling. Developmental history was significant for delays in sitting independently (8 months) and crawling (12 months), necessitating physical therapy for delayed gross motor milestones. Birth history and all other past medical history were unremarkable. Neurologic examination was positive for a mild left head tilt and lower extremity proximal muscle weakness on ventral suspension. Muscle bulk, tone, sensation, reflexes, and coordination were all within normal limits. MRI of the spine showed an expansile, enhancing, intramedullary mass from C2-T3 with an associated syrinx above and below the lesion (Fig 1). He subsequently underwent complete C3 to T3 laminectomies for spinal cord decompression and tumor resection.

The patient is an adult female who presents to the emergency department with complaints of nausea over the past few days and one day of non-bloody, non-bilious emesis. The patient was recently pregnant and delivered a baby girl a few months ago. Although she has been having difficulty keeping food and fluids down, she is continuing to breast feed her daughter. The patient endorses having a cold about a week ago, but denies any new foods, recent travel, sick contacts, diarrhea, illicit drug use, or toxic ingestions. Physical exam is notable for dry mucous membranes and tachycardia. The patient is otherwise afebrile, with a blood pressure of 120/80 mmHg, heart rate of 126 beats per minute, respiratory rate of 16 breaths per minute, and an oxygen saturation of 100% on room air.

In the fall 2019 semester, the students of the Liberal Arts and Management Program class Black Markets: Supply and Demand explored many types of black markets and examined many perspectives related to such illicit markets. Through careful discussion and reading the students discovered four prevalent themes throughout the course: the role of government in creating the context for black market activity, elements of demand, elements of supply, and varying levels of social implications. The thirteen articles in this volume provide rich takes on these themes. We placed each article with the theme we believe it most exemplifies; however, each article conveys facts and context that relate to each theme. We believe that these themes interact and work together like strands of a rope strengthening each other. Please note that authors of a couple of the articles personally observed others engaging in illicit activities. The authors did not. And the authors have not revealed true names of the persons they observed.
